Please note that there is already a report up from Pablo for the player side warn portion. This was made strictly for looking into and reporting the charge of staff abuse.
As the others have said, this is a very low effort appeal and makes no attempt to reassure the community that you will not repeat this behavior.

Yeah since they don't populate in our system, there's not much we can really do about it. I'm going to mark this as accepted though because again, you're not showing as banned in our system.

For multiple racism incidents/toxicity. Going off of your record it looks like the ban is in line with that. This ban is almost 2.5 years old, so I'd say it's a little late to claim it's false.

Thank you for the report. Appropriate action against the player and or staff member will be taken to ensure that they receive the correct punishment.
Off hours does not give a free pass to break rules. It's very disheartening to see a brand new tmod going out of their way to RDM someone who clearly wasn't happy about being shot. The way American brings up Chineseman talking in staff chat in a mocking way further reinforces that it was something more than just playing around. This is very disappointing and I hope it's just a one time incident.
You're fine, mistakes happen. The best thing to do is to learn from them, take a deep breath, and to move on. Dwelling on them will only serve to harm you.
