What bug is occurring? Thaddeus has the chance of teleporting to places that dont exist anymore such as the old Storage in EZ  Is it tied to a specific job, swep, player model, etc? Teleportation Device, Dr. Thaddeus Xyank  Has it only occurred once, or is it continuous?
